ILS – (DEED) Referral Form
We provide both I.H.S. w/ Training and I.H.S. w/ Family Training. Both can be provided in-person and remote. Independent Lifestyles ensures a remote service plan is in place to maximize goals that are being completed remotely.
Common goals under this service are:
- Learning how to cook independently while determining what ingredients are needed and what steps are needed to complete a meal.
- Learning how to manage finances independently.
- Building healthy relationships by learning positive behaviors, practicing appropriate conversations and identifying risk behaviors.
Independent Lifestyles does not permit transportation unless it is directly related to outcome building.
Our staff works directly with the consumer to teach these new skills with hands-on instruction. Together they write an independent living plan to meet individual needs, such as self-esteem, meal planning, financial management, obtaining housing and transportation, and much more.
Waivered In-Home Support Services include: In-home family support, Independent Living Skills training, Supported Living Services for Adults and Individualized Home Supports.
Independent Living Skills Assessment
Independent Lifestyles recognizes that consumers have a variety of IL needs as well as differing abilities and barriers that may keep them from living as independently as possible. Independent Lifestyles designed the Independent Living Skills Assessment. This evaluation tool is comprehensive, asset-based, and aims to highlight the consumer’s strengths and abilities. The assessment is based on consumer accomplishment of tasks through direct and indirect observation, self-assessment, and interviews. The following categories are included on the assessment:
- Personal Hygiene
- Dressing and Clothing Care
- Health Care
- Cooking, Eating, Nutrition
- Home Management and Home Safety
- Financial Management
- Personal Growth, Awareness, and Problem Solving
- Community Access
Upon completion of the assessment, a thorough summary of the evaluation and recommendations are provided. The assessment is used only as a guide and may be altered depending on the individual needs of the consumer. The Independent Living Skills Assessment may be purchased in full or as selected groups of modules to fit individual needs.
