Consumer Support Grants

Working with Counties, a monthly subsidy is provided to children and adults with disabilities living at home to purchase a variety of goods, supports and services beyond the usual home care services to maintain safe, independent living. 

This service provides Consumers flexibility, freedom and responsibility of choice in how they get their supports and services as well as who provides them. 

Staff assist Consumers in developing a person–centered plan to manage and pay for supports so they may continue to live in their homes.
