Individuals must be adults with a serious and persistent mental illness (SPMI).
By utilizing a Functional Assessment and Individual Community Support Plan, our case managers will work with the individuals to assess, address and monitor the effects of the daily instabilities and social determinants of health.
The referred individual must have active medical assistance and a recent diagnostic assessment. If a renewed diagnostic assessment is needed, the person served can work with the case manager to obtain a new assessment within 30 days of service initiation.
*We are able to serve eligible members with the following PMAPs: Blue Plus, Medica, Health Partners and UCare.
