Health Care Program

Comprehensive Care Coordination for Enhanced Health and Independence

In partnership with Managed Care Organizations (MCO), we provide care coordination for individuals who are enrolled in a Special Needs Basic Care (SNBC) plan.

In addition to SNBC, our Care Coordinators also work with 3 additional products for Medica. These include ISNBC (Integrated Special Needs Basic  Care/Enhanced), MSHO (Minnesota Senior Health Options) and MSC+ (Minnesota Senior Care Plus).

Care Coordination

This portion of the service is a benefit that members enrolled in these products have access to. 

Our Care Coordinators work one-on-one with members enrolled in any of the above products to ensure their medical needs are being met and they have access to community supports and services

Our Care Coordinators also assist individuals to identify person centered goals and work with them throughout the year to achieve these goals. Through our health care program individuals can live a healthy and an independent lifestyle.

*Referrals for this program are received from Managed Care Organizations. If you’d like to receive Care Coordination services from Independent Lifestyles, please contact your Health Plan.
